My application is missing from my work place i started about a month ago, i noticed at my work that my?

co-workers are a little fishy and try to get away with as much as they can. i work at a sea food restaurtant. i need to know why my application would disappear out of nowhere after a month of me being there, on sunday ( 2days ago) the manager told me that it got lost and i need to fill out another one, hmmmm i just dont get what kind of scam theyre tryin to do and how can i defend myself from this situation?
