Parents w/special needs children What should my son's school be doing in reference to Transition Services ?

My son has a DX of Aspergers,Bipolar Disorder, Anxiety,non verbal learning disorder and Obesity."BUT" he is 17 and in the 11th grade but with modified instrution and materials he is able to complete gradeappropriatee ignments. which is a crock of bull he is doing it on his own in his High school the children with learning challenges are overlooked by the children with behavioral problems.anyways i was wondering what transitional services have you been offered uponn graduation ?
